Registration Forms

PLAYER ADD FORMS are available in the softball office, when office hours begin in April.

Please do not use the liability form from this page to add new players to your team.

Adding players:

Add-on forms are available during office hours.

Forms must be filled out and may be turned in anytime prior to playing a game

Please DO NOT use forms from this website to add players.

Players may be added up to and including your last regular season game, but still must qualify for any post season play.

 

FALL LEAGUE INFORMATION

8 game season with double elimination playoffs.

Tentative  dates are :

SUNDAY                      9/7-11/9

The rest of the league will begin play on the week of 8/25

MONDAY                     8/25-11/3

TUESDAY                    8/26-11/4

WEDNESDAY             8/27-8/5

THURSDAY                8/28-11/7

FRIDAY                        8/29-11/14 (no Halloween games)

Walnut Festival 9/16-21 no games

These dates include 2 weeks for playoffs

Dates may have to be adjusted for weather conditions.

Playoffs will be the top 50% of each night. Different nights of play will not be mixed due to the limited season. If a 8 team league then it will be the top 4.

Due to field use restrictions, we are limited to ten teams per night, so registration will be on a first come first serve basis. A team will only be accepted if their packet is complete with all fees paid.

The following forms (except individual player) are needed to make a compete packet

1. COVER SHEET -(Roster-Contract) bring completed form to registration.

The Cover sheet will need to be filled out on 3 part NCR at registration. A manager’s signature on that form is required

2. ROSTER LIST FORM - include all players on your team. Please fill out completely. +Incomplete information will delay your team’s registration.Remember W.C. residents will need to rpovide proof of residency.

3. LIABILITY FORMS – each player on your roster must sign a liability (waiver) attached to packet.

These may be faxed back and forth between players and managers to lighten the load for you

4 . REQUEST FOR NIGHT OF PLAY- two choices must be listed for teams. Teams may be placed on their second choice, for the sake of parity. On rare occassions a third may be offered, but placement is discussed with the manager before such a move is made.

There is no guarantee of first choice, but we do try.

5. INDIVIDUAL PLAYER FORM- If you are looking to get on a team, fill out form below and fax to number on sheet. We do not place players on teams, but do make the forms available to managers looking for players during softball office hours. The names are not for distribution.

 

Fees due at registration - no credit cards, monopoly money, shells or beads

Registration-$500.00 (per team)

Non-resident $11.00 (per non-resident-this is addition to the Assessment fee))

Assessment $5.00 (per player)

ALL OF THE FORMS MUST BE COMPLETE AND TURNED IN WITH PAYMENT AT REGISTRATION.

Cash, money orders or checks (or any combination) are accepted. Please make it only one check for the total.

UNDER THE DOOR OR THROUGH THE MAIL SLOT REGISTRATIONS WILL NOT BE ACCEPTED AND WILL BE RETURNED.
