Registration Forms

ADDING PLAYERS

PLAYER ADDITION FORMS are available in the softball office,

during office hours in April.

Please DO NOT use the liability form from this

page to add new players to your team.

PACKETS ARE NOT MAILED-FORMS MUST BE DOWNLOADED. This helps in reducing the cost for the league. We do appreciate your cooperation.

 

The following forms (except individual player) are needed to make a compete packet

1. COVER SHEET -(Roster-Contract) bring completed form to registration.

The Cover sheet will need to be filled .The manager’s signature or team representative on that form is required. The 3 part NCR will no longer be used.

A receipt will be issued for all teams at registration

2. ROSTER LIST FORM - include all players on your team. Please fill out completely. Incomplete information will delay your team’s registration.

Remember W.C. residents will need to provide proof of residency. A copy of CDL, utility statement, credit card statement will do. These are returned to managers.

If proof is not presented at registration a non-refundable non-resident fee will apply

3. LIABILITY FORMeach player on your roster must sign a liability (waiver)

These may be faxed back and forth between players and managers to lighten the load for you

4 . REQUEST FOR NIGHT OF PLAY- three choices must be listed for teams. Teams may be placed on their second or third choice, for the sake of parity. On rare occassions a third may be offered, but placement is discussed with the manager before such a move is made.

There is no guarantee of first choice, but we do try.

5. INDIVIDUAL PLAYER FORM- If you are looking to get on a team, fill out the form below and fax to number on sheet. We DO NOT place players on teams, but do make the forms available to managers looking for players during softball office hours. Another way to join a team is to come to the fields during play, after 6:00 pm M-F (4:00pm Sundays) and talk to managers.

 

FEES DUE at REGISTRATION - no credit cards, cash or checks only. One check per team, made out to W.C.A.S.I . There is a $15.00 charge for returned checks.

Registration- Fees TBD for 2011

Non-resident $11.00 (per non-resident-this is addition to the Assessment fee))

Assessment $6.00 (per player)

ALL OF THE FORMS MUST BE COMPLETE AND TURNED IN WITH PAYMENT AT REGISTRATION.

Cash, money orders or checks (or any combination) are accepted.

Refund policy:Full refund is cancelled after registration and prior to teams posted.

Partial refund (75%) if cancelled after registration and prior to regular season schedule posted.

Partial refund (50%) if cancelled after after regular season scheduled posted.

No refund due after regular season has begun

UNDER THE DOOR OR THROUGH THE MAIL SLOT REGISTRATIONS WILL NOT BE ACCEPTED AND WILL BE RETURNED.
